What is a nursing brain sheet?

A brain sheet, also called a nurse report sheet, is the page a nurse fills in during handoff and carries all shift to track each patient's diagnosis, meds, labs, lines and tasks.

Is this a brain sheet or a report sheet?

Both names describe the same page. Nurses say brain sheet, report sheet, and cheat sheet interchangeably, so an LTC nurse report sheet, an SNF nursing brain sheet, and a long term care report sheet template are all this: one sheet you fill in at handoff and carry through the shift.
